Salary: $17+ DOE
The case manager reports directly to the program manager. He/she will assist clients to access needed medical, educational, social, prevocational, vocational, rehabilitative, or other community services. They will focus on coordination of SUD care, integration around primary care especially for clients with a chronic substance use disorder, and/or co-occurring conditions, and interaction with the criminal justice system, if needed. Some of the case managers' duties will include; comprehensive assessment and periodic reassessment of individual needs to determine the need for the continuation of case management services; revision of client treatment plan when necessary, actively helping clients to apply for and maintain health and public benefits, provide direct services to clients, including one-on-one meetings, to facilitate referrals in support of clients' treatment plan goals, and maintain regular progress notes, records, data entry, and other documentation as required, that meets DMC standards.
Qualifications, Skills, Knowledge, and Experience: Must be a certified/registered alcohol and drug counselor in compliance with all DHCS standards licensed and/or licensed eligible California Board of Behavioral Sciences. Past experience should demonstrate at least two years of behavioral health experience; proven ability to work across multiple systems such as Probation, CWS, DDC, criminal justice, etc.; and strong knowledge of and relationships with community resources. Skills include excellent written and verbal communication, ability to manage multiple tasks in a fast-paced environment, strong documentation, and proven success as a team player. Must have a clean driving record with a minimum of two years driving experience. Must comply with all agency and contract requirements related to trainings, public health, certifications, reporting, etc. Individuals with bilingual proficiency preferred.
