Be part of an excellent care team: Under the clinical supervision of the Integrated Behavioral Health Department, the SUD Counselor is an essential member of the primary care team who focuses on facilitating and improving care to clients requiring assistance dealing with substance mis-use, such as opioid use disorder, alcohol or drug abuse. Services are provided at La Clínica's primary care clinics in Alameda, Solano, and Contra Costa County, as well as over the phone. The SUD Counselor is most suited to a substance abuse/alcohol and drug/addiction certified individual with a BA and two years of experience in a medical/social service setting providing SUD services. Additionally, the individual in this position will have relevant experience and/or training in residential chemical dependency treatment.

Major Areas of Responsibility include but are not limited to:

Essential job duties

  • Provide substance abuse counseling to clients requiring assistance dealing with substance mis-use, such as opioid use disorder, alcohol use disorder, and stimulant misuse.
  • Provides assessment, supportive SUD counseling and case management to clients in an outpatient setting. Conducts intake of new clients in compliance with regulatory standards. Provides direct services to clients and completes all required clinical documentation in accordance with organization requirements.
  • Provide a safe and therapeutic environment by monitoring patients and referring to more intensive behavioral health services when needed.
  • Develops effective multidisciplinary treatment plans for patients. Assumes case management responsibility for assigned patients.
  • Assist patients in initiating medication assisted treatment in coordination with patient's primary medical provider and care team.
  • Act as client advocate in order to coordinate required services or to resolve urgent medication issues.
  • Act as a single point of contact to receive referrals, from medical providers and behavioral health clinicians
  • Support patient engagement, linkage, coordination, monitoring, follow-up and advocacy
  • Maintain close communication with related health, educational, behavioral health and social service agencies
  • Promote timely and coordinated internal communications among patients, clinic primary care providers, behavioral health clinicians, case managers, consulting/treating psychiatrists and others involved in the patient's care provision
  • Facilitate warm hand offs between medical and behavioral health staff and provide referrals for community resources to support whole person health.

Reporting

  • Use Electronic Health Record (EHR) and population management tools to collect data, communicate with medical and behavioral health providers and produce reports to support collaborative care, improved health outcomes and program compliance
  • Monitor participation in primary care and behavioral health care and provide outreach, advocacy, referral, linkage and follow-up through phone calls, meetings, and coordination in order to effectively and efficiently link SUD patients to all levels of assessment and care within both primary care and behavioral health systems of care. Follow up on no-shows

Outreach & Advocacy

  • Attend external meetings as needed to ensure the coordination of care for SUD patients
  • Involve family members/significant others as necessary to increase patient participation and improve patient well-being
  • Establish and maintain good working relationships with staff and medical providers; participate in case presentations, seminars, supervision, meetings, trainings, peer review as needed.

Knowledge

  • Cultural competence working in diverse/low income and immigrant communities.
  • Knowledgeable in the principles, practices and techniques of social work; basic psychological theory and practices as related to human behavior
  • Familiarity with medication assisted treatment (MAT) approaches
  • Familiarity with Motivational Interviewing required.

Abilities

  • Ability to quickly identify risk factors as they relate to patient safety and communicate these immediately and effectively to appropriate team members.
  • Ability to form empathic and collaborative relationships with patients with medical and behavioral health challenges and flexibility in meeting patients' needs
  • Excellent organizational skills and a high degree of interpersonal competence with both staff and patients
  • Ability to work with others on the startup of a new program
  • Ability to work in a fast-paced environment as part of an interdisciplinary team, and to establish effective working relations with staff, patients and community partners
 

Education, Other Certifications and Experience

  • Bachelor's Degree from an accredited program or university preferred
  • Drug and Alcohol Certification recognized by Department of Health Care Services (DHCS).
  • Bilingual in Spanish and English Required
  • Relevant experience and/or training in residential chemical dependency treatment preferred
  • Experience providing support and advocacy
  • If in recovery, three years of sobriety required.
