The Addictions Counselor is a vital member of the Integrated Health Team and works closely with other behavioral health professionals and medical staff. This position is primarily responsible for screening, evaluating, and treating patients for problematic substance use and addiction. Our addictions treatment program is ASAM level I program including individual and group therapy, with access to primary care psychologists, tobacco cessation specialist, dietician, and psychiatrist. We work collaboratively with community addictions IOP, Residential Treatment, Drug Court, and Parole/Probation. Our outpatient treatment program is embedded in a primary care setting in accordance with the applicable professional and ethical standards. Within the health center, patients can access adult medicine, OB/GYN, and dental services, and support from Women, Infant, and Child (WIC).

Candidates must demonstrate excellent interpersonal skills, a commitment to assisting patients to achieve recovery, competency to provide established addictions evidence-based practices, familiarity with community support groups, ability to multitask, comfort with an electronic health record, an ability to complete documentation in a timely fashion, and skills to work collaboratively within the health center as well as in the community.

Essential Duties and Functions:

  • Conducts initial substance use assessments, continuing care determinations, treatment planning, referral to community resources, and discharge planning based on ASAM criteria.
  • Provide individual and group counseling to individuals with problematic substance use, addiction, and co-occurring mental health problems.
  • Utilizes Motivational Interviewing, Stages of Change, Integrated Dual Diagnosis, Harm Reduction, Relapse Prevention, and Cognitive Behavioral Interventions. Familiarity with the Anger Management, Matrix Model, 12-Step Facilitation, Seeking Safety, Dialectical Behavioral Therapy, and treatment of opioid use disorder are beneficial.
  • Provides an overview of CCM’s outpatient treatment program to the patient, describing the nature and goals of the treatment, rules of the program, the patient’s rights and obligations, and program hours and services.
  • Educates the patient and family about problematic substance use, addiction, and co-occurring mental health problems. Provides support, therapy, and connects with community resources when indicated.
  • Develops crisis intervention plans collaboratively with patients to ensure safety and sobriety.
  • Consults with CCM health care providers and outside professionals to assure comprehensive treatment and quality care for the patient.
  • Makes appropriate referrals to CCM health care providers and programs, as well as to outside agencies, and works cooperatively with other providers to facilitate integrated care as appropriate.
  • Documents assessment and clinical services to ensure medical necessity of treatment, collaboration with other providers, and referrals to community resources.
  • Obtains signed authorization forms from the patient for the appropriate release of information for outside providers/sources. Obtains signed informed consents for the outpatient treatment from the patient.
  • Maintains thorough and confidential patient records, clearly and timely documenting all appropriate findings, diagnoses, assessments, treatment plans, and recommendations. 

The successful candidate must have experience and training as a Certified Substance Abuse Counselor (CSAC) in the State of Hawaii within 1 year of employment. Supervision and support for training towards achieving CSAC may be provided. Preferred, but not required, credentials are a Master’s degree in psychology, counseling or social work. There may be support to achieve Hawaii State licensing as a master’s level mental health professional.

The ideal candidate currently resides on Maui or will relocate to Maui within 8 weeks upon acceptance of employment. Full-Time, Exempt position reports to the Integrated Health Director in our Wailuku location.
