Assist with admission into SUD services, transitioning from one level of care (LOC) to another, navigating the mental health, physical health and social service systems and carry a case load as assigned by program management. Communicate with a network of providers as participants move between LOC’s and into post- discharge recovery services to support successful transitions back into the community. Provide individual and group counseling/case management, regular assessments, and crisis intervention in accordance with the approved Evidence Based Practices (EBP). Provide evaluation; plan development; placement services; assistance in daily living; linkage and consultation to Substance Use disordered clients. Provide referrals to adjunct services in relapse prevention, independent living skills training. Provide outpatient follow up services upon discharge.
Experience:
Minimum of two years’ experience in substance use disorders in a mental health or substance abuse setting preferably with homeless,forensic, dual diagnosed individuals. Experience in individual and group counseling. Two years alcohol & drug treatment experience preferred.
