Case managers are integral members of a multi-disciplinary team that providers a continuum of services to participants under Probation Supervision. The Case Manager provides one-on-one services to adult participants and is the primary coordinator of service delivery, including facilitation of evidence-based groups. Other responsibilities include developing Individual Service Plans with participants and ensuring follow through on linkages to mental health, substance abuse treatment and other outside services.

Full-Time Substance Abuse Case Manager Responsibilities:

• Provide one-on-one services to ADRC clients and facilitates groups;

• Develop an Individualized Treatment Plan for each client, and provide services to caseload of clients;

• Ensure follow through on linkages to mental health, substance abuse treatment and other outside services and provide support;

• Participate in the intake of clients as required; conduct initial interviews of clients assigned to the caseload, and provide program orientation;

•Facilitate the substance abuse rehabilitative program groups;

• Conduct individual, small, and large group counseling sessions;

• Establish and verify a comprehensive daily schedule for clients based on the case plan, including treatment and leisure time schedules;

• Make recommendations regarding caseload matters;

• Conduct and participate in case conferences for each client assigned to the caseload;

• Provide updates of client performance at case conferences with APD;

• Follow the program lesson plan/curricula to ensure fidelity of treatment;

• Attend all staff meetings;

Minimum qualifications and experience:

A minimum of 2 years of experience working with the criminal justice, homeless, or substance abusing population. Applicants must be a registered Alcohol and Drug Technician (RADT) or an Associate Degree or the equivalent related experience.

Desirable characteristics: Case Managers must be well organized, have strong oral and written communication skills, and have cross-cultural sensitivity and competency. They possess integrity, intelligence, enthusiasm, a sense of humor, and an understanding and respect for the criminal justice population.

Regular work days and hours: Monday through Friday from 8:00am to 5:00pm.

Salary: $18.00 to 20.00 per hour DOE
